Although Hodgkin's disease HD ; is considered a highly curative neoplasm, about one-third of all patients fail to respond to conventional chemotherapy alone or combined with radiotherapy. Infusion-related back pain was reported exclusively by AMD patients treated with Visudyne therapy at an incidence of 2.0%. It was reported as mild or moderate in severity in all incidences, with the exception of one case in which Visudyne therapy was discontinued. The mechanism of infusion-related back pain is not clear. It has not been associated with any evidence of hemolysis, allergic reaction or renal toxicity, and usually resolved by the end of the infusion. When patients were rechallenged with infusion, outcomes were mixed and there were several instances when no back pain was reported. The patients included in the pathologic myopia arm of the VIP Trial and in the VOH Study did not report any back pain and were, on average, younger than the AMD patients in the TAP Investigation and VIP Trial. Photosensitivity reactions Photosensitivity reactions were reported in 2.2% of patients treated with Visudyne therapy. These were generally mild to moderate transient reactions that developed from direct exposure to sunlight within 3 days of treatment and resolved within 1 week Table 6.3 ; . Only two severe reactions were reported and both occurred on the day of treatment. Photosensitivity reactions rarely occurred in the VIP Trial despite the shorter light-protection period of 24 hours compared with the 48 hours used in the TAP Investigation. This strongly suggests that the reduction in photosensitivity reactions is related to better education and compliance by the patients and not to the length of the light protection period.

Bleomycin is a new cytotoxic antibiotic elaborated by a mutant strain of the soil bacterium, Streptomyces verticillus 10 ; . It was isolated in Japan during a search for antibiotics with an antitumor spectrum similar to that of phleomycin but lacking its nephrotoxicity. The structure of bleomycin has not been fully elucidated, although it can be separated into 2 active substances, bleomycins A and B, which can be further fractionated into a number of peptides containing amino acids, sulfur, and sugars, some of which are quite unusual 12 ; . The drug appears to act by means of single-strand scission of DN A Bleomycin inhibits the growth of a number of both gram-positive and gram-negative bacteria 11 it has also demonstrated an effect against a wide spectrum of animal tumors, including both solid and ascitic varieties 11 ; . Recent clinical reports have shown the efficacy of bleomycin against well-differentiated squamous cell carcinomas involving the skin, head and neck, larynx, esophagus, penis, lung, and cervix, as well as against testicular neoplasms, melanoma, and lymphomas, especially Hodgkin's disease 1 ; . This drug is of particular interest, not only because of its activity in certain tumors that have heretofore been resistant to treatment, but also because of its peculiar toxicity. In contrast with the effects of other cytotoxic chemotherapeutic agents, depression of the hematopoietic system has only rarely been reported. Thus, the potential exists for the use of.

Effects of bleomycin on rate of spheroplast formation. We confirmed the cell wall effects of bleomycin treatment by assessing rates of spheroplast formation. Cells were incubated overnight in growth medium in the presence or absence of bleomycin 25 , ug ml ; , washed, and converted to spheroplasts. Drug-treated and control cells were harvested, washed, and treated with 10 mM DTT to increase cell wall permeability or treated with water. The cells were washed again and incubated with a lytic enzyme mixture containing p1 -- 3 glucanase Lyticase ; to disrupt the cell wall 10 ; . Table 1 shows that incubation in the presence of the drug increased the rate of spheroplast formation at least 10-fold. Bleomycin facilitation of spheroplast formation was also observed when the cells were treated with bleomycin for 1 h.
