To determine if a sexually transmitted disease or some associated factor might be responsible for the observed association between nonerythromycin macrolides and pyloric stenosis, we estimated the rate of pyloric stenosis among infants of mothers exposed to doxycyline, an antibiotic used for similar indications as the nonerythromycin macrolides. Among the 1635 pregnancies associated with doxycyline use, there were four children with pyloric stenosis 2.45 per 1000 ; , a prevalence similar to controls 2 0.11, P .74 ; . Ann Intern Med 107: 78-87, 1987 Progressive multifocal leukoencephalopathy, a common complication of infections with human immunodeficiency virus HIV ; , occurs in as many as 3.8% of patients with the acquired immunodeficiency syndrome AIDS ; . We report 16 cases and review 12 previously reported cases of progressive multifocal leukoencephalopathy associated with HIV infection. This illness was the presenting manifestation of HIV infection in 8 cases. Limb weakness, gait abnormalities, visual loss, aand altered mental status were the commonest initial complaints. Computed tomography of the brain frequently showed hypodense, nonenhancing white matter lesions. Magnetic resonance imaging was more sensitive than computed tomography in detecting lesions. Cerebrospinal fluid analysis and electroencephalography were nondiagnostic. Impaired cell-mediated immunity was typically noted, even in the absence of other immunodeficiency-associated illnesses. Death occurred within 10 days to 18 months of the onset of symptoms in 22 patients. However, 4 patients remain alive at 3 to months; of these 4, 2 have had significant improvement without treatment. These particles and rays produce their effect by ionizing the chemical compounds that make up the living cell. If enough of these particles or rays disrupt a sufficient number of molecules within the cell, the cell will not be able to carry on its normal functions and will die. ALPHA.--Alpha particles are emitted from the nucleus of some radioactive elements. Alpha particles produce a high degree of ionization when passing through air or tissue. Also, due to their large size and electrical charge, they are rapidly stopped or absorbed by a few inches of air, a sheet of paper, or the superficial layers of skin. Therefore, alpha particles do not constitute a major external radiation hazard. However, because of their great ionization power, they constitute a serious hazard when taken into the body through ingestion, inhalation, or an open wound. BETA.--Beta particles are electrons of nuclear origin. The penetration ability of a beta particle is greater than an alpha particle, but it will only penetrate a few millimeters of tissue and will most probably be shielded out by clothing. Therefore, beta particles, like alpha particles, do not constitute a serious external hazard; however, like alpha particles, they do constitute a serious internal hazard. NEUTRONS.--Neutrons are emitted from the nucleus of the atom. Their travel is therefore unaffected by the electromagnetic fields of other atoms. The neutron is a penetrating radiation which interacts in billiard-ball fashion with the nucleus of small atoms like hydrogen. This interaction produces high-energy, heavy-ionizing particles that can cause significant biological damage similar to that produced by alpha particles. GAMMA RAYS.--Gamma rays are electromagnetic waves. Biologically, gamma rays are identical to x-rays of the same energy and frequency. Because they possess no mass or electrical charge, they are the most penetrating form of radiation. Gamma rays produce their effects mainly by knocking orbital electrons out of their path--thereby ionizing the atom so affected--and imparting to the ejected electron. Neutrons and gamma rays are emitted at the time of the nuclear explosion, along with light. Gamma rays and beta particles are present in nuclear fallout along with alpha particles from unfissioned nuclear material. In the present study, we hypothesized that aldosterone MR may stimulate collagen synthesis through the activation of ERK1 2 in renal fibroblasts. To test this hypothesis, we first investigated whether MR is expressed in cultured rat renal fibroblasts. Next, we examined the effects of aldosterone on ERK1 2 phosphorylation, as well as collagen gene expression and synthesis in the renal fibroblasts. Finally, we examined the effects of eplerenone and PD98059, a specific inhibitor of mitogen-activated protein kinase ERK kinase, which is the upstream activator of ERK1 2, 14 on the aldosterone-induced collagen gene expression and synthesis and erbitux.
ABSTRACT . The pathophysiological consequences of excess mineralocorticoid for salt status include hypertension, vascular inflammation and cardiac fibrosis. Mineralocorticoid receptor MR ; blockade can both prevent and reverse established inflammation and fibrosis due to exogenous mineralocorticoids or endogenous glucocorticoid activation of the MR. Glucocorticoids also exert potent anti-inflammatory effects via glucocorticoid receptors GR ; in the vascular wall. We propose that GR signaling may ameliorate mineralocorticoid salt-induced vascular inflammation and fibrosis in the mineralocorticoid salt model. In the present study the role of GR in the mineralocorticoid salt model was explored in uninephrectomised rats, maintained on 0.9% saline solution to drink and treated as follows: control, no further treatment CON deoxycorticosterone DOC; 20mg week ; for 4 weeks DOC4 DOC for 8 weeks DOC8 DOC for 8 weeks plus the GR antagonist RU486 2mg day ; weeks 5-8 DOC8 RU486 ; , and; DOC for 8 weeks + plus RU486 and the MR antagonist eplerenone 50mg kg day ; for weeks 5-8 DOC8 RU486 + EPL ; . DOC treatment significantly increased systolic blood pressure, cardiac fibrosis, inflammation ED-1-positive macrophages and osteopontin ; and mRNA for markers of oxidative stress p22phox, gp91phox and NOX-4 ; . GR blockade reduced the DOC-mediated increase in SBP and the number of infiltrating ED-1-positive macrophages but had no effect on fibrosis, oxidative stress or osteopontin mRNA levels. Eplerenone reversed DOC-induced pathology in the absence or presence of GR blockade. Thus blocking agonist activity at the GR neither enhances nor attenuates the fibrotic response, although it may modulate SBP and macrophage recruitment in the mineralocorticoid salt model.

| To determine whether mineralocorticoid receptor MR ; activation plays a role in diabetic renal injury and whether this role differs in types 1 and 2 diabetes mellitus, we examined the effect of a MR antagonist on renal injury in rodent models of type 1 streptozotocin-treated rat ; and type 2 db db mouse ; diabetes. We studied three groups of 8-wk-old, uninephrectomized Wistar rats for 4 wk: diabetic streptozotocin- 55 mg kg ; treated rats n 11 ; , diabetic streptozotocin-treated rats receiving the MR antagonist eplerenone n 15 ; , and nondiabetic rats n 9 ; . addition, we studied three groups of 8-wk-old mice for 16 wk: diabetic db db mice n 10 ; , diabetic db db mice treated with eplerenone n 8 ; , and nondiabetic, db littermates n 11 ; . Diabetic rats and mice developed albuminuria and histopathological evidence of renal injury, including glomerular hypertrophy, mesangial expansion, and tubulointerstitial injury as well as increased renal cortical levels of MR protein, MR mRNA, TGF mRNA, and osteopontin mRNA. All of these changes were significantly reduced by treatment with eplerenone except for the elevated MR levels. The beneficial effects of eplerenone were not attributable to changes in blood pressure or glycemia. In summary, MR expression was increased in kidneys of diabetic rodents, and MR antagonists effectively reduced diabetic renal injury irrespective of the species or specific cause of the diabetes. Thus, these data suggest that MR activation is a critical factor in the early pathogenesis of renal disease in both type 1 and type 2 diabetes mellitus.
