Cell cultures using procedures previously described.15 The protocol chemotherapy schema is shown in Fig 1A. Patients were first treated with four 2-week cycles of MLF, which consisted of sequential methotrexate 250 mg m2 intravenous [IV] at hour 0 ; , 5-fluorouracil 500 mg m2 IV at hours 4 and 25 ; , and leucovorin 250 mg m2 at hour 24 ; . At least 14 days after the last MLF cycle, when the absolute neutrophil count ANC ; exceeded 1, 200 L and platelets exceeded 90, 000 L, patients were then treated with two 3-week cycles of cyclophosphamide 4 g m2 over 4 hours ; with mesna 2-mercaptoethanesulfonate uroprotection. Patients received filgrastim 10 g kg beginning on day 2 of the first cyclophosphamide cycle and underwent apheresis when the white blood cell WBC ; count increased to greater than 5, 000 cells L after reaching the nadir level. Peripheral blood progenitor cells were harvested during recovery from cyclophosphamide using a Fenwal CS3000 Baxter Corp, Deerfield, IL ; or a Cobe Spectra COBE, Lakewood, CO ; apheresis device. All patients underwent two apheresis procedures each, with a mean of 15.2 L of blood processed per procedure. Adequate numbers of PBPCs were collected in all gene-therapy patients after the first cyclophosphamide cycle. The collected mononuclear cells were CD34 enriched using a Ceprate stem cell concentrator column Cellpro, Bothell, WA ; according to the manufacturer's instructions.16 An aliquot 5 106 ; of CD34-selected cells was examined for breast cancer cell contamination by immunohistochemistry using a panel of anti-keratin antibodies. All of the CD34 samples were negative for tumor cell contamination, with the limit of 106 detection of 1 tumor cell in 105 CD34 cells. At least 1.5 CD34-selected cells per kilogram were stored without further manipulation for hematopoietic rescue in the event of engraftment failure. These cells were placed in freezing medium containing 50% human AB serum, 40% Plasmalyte-A Baxter ; , 10% dimethyl sulfoxide DMSO.

National lists should not use a similar symbol and should be specific in their final selection, which would depend on local availability and price. NYHA functional class FC ; and 6MWT are used as endpoints and to guide individual patient therapy. 6MWT is fairly reproducible, FC is subjective in the mid-range Classes II & III ; . In patient management, this subjectivity can impact therapy. In studies, endpoints might be impacted. Consistent functional classification is expected by the same observers assessing the same patients in these studies. Change in one assessment should theoretically be in the same direction as the other. In PAH studies, discrepancies have been observed . Our study aims to assess the sensitivity and specificity of a change in the 6MWT to predict a change in NYHA FC. METHODS: Data was derived from the STRIDE-1 study. 178 PAH patients were enrolled testing the safety and efficacy of Sitaxsentan. 6MWT and FC were recorded at baseline, 6 and 12 weeks. A change in 6MWT was defined 10% from baseline. Complete data was available on 168 patients. RESULTS: 45 patients had more than 10% improvement in their 6MWT, 19 improved their FC, none worsened, 26 did not change their FC. Sensitivity of a 10% and more change in 6MWT to predict FC improvement is 48% with a specificity of 73%. 31 patients had more than 10% deterioration in their 6MWT, 2 worsened their FC, 1 improved and and methadone.
