Protease inhibitors Aprotonin: Aprotinin is a serine protease inhibitor which inhibits a wide range of proteases including trypsin. It is a member of the Kunitz family of proteins, now called bikunins and has been used during coronary artery bypass surgery as an anti-fibrinolytic agent. In one study, aprotinin administered intraveniously did not prevent post ERCP pancreatitis[59]. C1-INH: In a pilot study, C1-INH, a potent inhibitor of the first step in the complement cascade was given intravenously to 20 patients in a dose of 3000IU 30 min prior to ERCP. There was a significant reduction in post ERCP hyperamylasemia in the C1-INH group compared to the placebo group. This study did not assess the prevention of post ERCP pancreatitis[60]. Ulinastatin: In an uncontrolled study from Japan, ulinastatin a protease inhibitor isolated from human urine was found to show promising results when given as continuous arterial infusion in severe acute pancreatitis[61]. Another Japanese study on 406 patients found a significant reduction in post ERCP hyperamylasemia and pancreatitis in patients administered 150 000U of ulinastatin , compared to placebo[62]. Gabexate: The story of gabexate, a synthetic protease inhibitor which can act on trypsin, phospholipase A2, kallikrein, plasmin, thrombin, and C1 esterase, is a classic example of the efforts made to prevent post ERCP pancreatitis. Gabexate was synthesized in 1977 [63] and was first used as a prophylaxis agent against ERCPinduced pancreatitis in 1982[21]. In some studies, gabexate administered over 4 h 300 mg ; to 12 h 1 gram started 30 min before ERCP ; was found to reduce post ERCP pancreatitis[64, 65]. However, other studies failed to confirm these results but a meta-analysis found gabexate to be useful when given for at least 6 h[66]. To further muddy the waters, a subsequent large, multicenter trial on 1127 patients using a 6.5 h infusion period found no difference between gabexate, somatostatin, and placebo [67], and a recent large meta-analysis likewise found no benefit of gabexate[68]. Unfortunately, no pharmacological agent has been shown to definitively reduce the risk of post ERCP pancreatitis. Further studies with NSAIDS and 5FU are needed but are unlikely to be successful.

NHE3 appears to be the predominant brush-border NHE isoform in mammals. This important role of NHE3 exchange is demonstrated in NHE3 knockout mice, which exhibit chronic diarrhea and disturbances in systemic fluid balance 22, 38 ; . Therefore, we initially investigated the effects of NHE3 inhibition with S-3226 in the ischemia-injured ileum. However, application of S3226 had no influence on TER, despite causing significant reductions in Na Jms. Inhibition of NHE3 has been shown to be involved in regulation of TER. Turner et al. 43 ; showed that inhibition of NHE3 with S-3226 induced elevations in TER that occurred only in the presence of activated Na glucose cotransporter in Caco-2 cell monolayers. However, in our Ussing chamber model, glucose is selectively added to the serosal side of mucosal sheets to prevent activation of this pathway, and thus the findings of Turner et al. may not be entirely relevant to our observations in the ischemia-injured mucosa. Born in Athabasca, Jonson attended university and then taught school in rural communities until 1969, when he moved to Ponoka to take a position as a school viceprincipal, later becoming principal. As he came to know his students, he also became increasingly aware of how many people lived and worked at the mental hospital and that most of the people in the area had some relationship to the region's largest industry--the Alberta Hospital. Following a term as president of the Alberta Teachers' Association, Jonson tried his hand at provincial politics, and, in 1982, he was elected to the Alberta Legislature by a huge margin. He maintained a strong constituency association and never forgot the importance of building and maintaining good relations with hospital staff--and, of course, the local businesses dependent on hospital paycheques. Civil servants responsible for Ponoka were quite aware of the MLA's interest in and loyalty to the hospital and decisions were made with great sensitivity to his influence. The government had, for many years, been sensitive to its rural voters and Ponoka Rimbey was staunchly conservative. Any Member of the Legislature's view about his or her constituency was critically important and even in the early days of Premier Lougheed's term, and in spite of the Premier's commitment to mental health reform, the local MLA's views were usually paramount in any discussion. Peptide-acetaldehyde adducts were separated by reversed phase HPLC. Fast atom bombardment FAB-MS ; and linked scan B E ; spectra were used to study adduct structures. Under nonreducing conditions at pH 7.4 1: 10 peptide: aceta!dehyde ratio ; two adducts of m z 824 were formed with modifications at the N-terminal valine, a Schiff base and a cyclic 2-melhyl-imidazolidine-4-one. The 2-methylimidazolidine-4-one adduct formed via the Schiff base and dissociated gradually after 24h. Reducing conditions at pH 7.4 1.1.100 peptide: acetaldehyde: NaCNBH3 ratio ; resulted in the formation of an adduct of m z 826, an N-ethylated adduct of valine A small amount of nonreduced adducts were also formed under these conditions. Reducing conditions at pH 9.0 1: 10.100 peptide: acetaldehyde: NaCNBH, ratio ; yielded two diethylated m z 854 ; products. Trie cysteine residue of the peptide was not found to form an adduct with acetaldehyde under any physiologically relevant condiuons. A structural characterization of two nonreduced adducts, a Schiff baie and a cyclic 2-methyl-imidazolidine-4-one, and one reduced, ethylated adduct of Ntemunal valine of hemoglobin 3-chain at physiological pH is presented in this and metolazone.
The ischemic zone. Our data indicate that improved cellular electrical activity of endocardium and Purkinje fibers can occur in an environment of constant hypoxia, hyperkalemia, and acidosis, even without exogenous glucose and free fatty acids. Although we did not observe significant recovery of electrical activity in separate pieces of epicardium, endocardial improvement possibly could contribute to the recovery of epicardial activity during myocardial ischemia in vivo. The data for three preparations in which impalements were maintained throughout successive superfusions of altered Tyrode's solution suggest that less marked changes occurred during the second exposure to altered Tyrode's solution than during the first exposure. This observation may represent a cellular correlate to the observation that the initial occlusion of a coronary artery produces more severe electrical changes than subsequent occlusions Boineau and Cox, 1974; Ruffy et al., 1977; Scherlag et al., 1974 ; . Superfusion with solution containing an elevated potassium concentration in vitro, however, may have merely replenished potassium losses incurred during dissection and equilibration of the preparation Langer and Brady, 1966 ; and resulted in more negative resting potentials and a diminished response to the depolarizing effects of subsequent superfusion with altered Tyrode's solution. Effects of Verapamil and Tetrodotoxin Our data concerning the effects of verapamil on canine Purkinje fibers are in agreement with previous studies Cranefield et al., 1974; Rosen et al., 1974 ; . Shortening of APD50 apparently is the result of slow-channel blockade, whereas prolongation of APD90 results from depressant effects on outward plateau current Kohlhardt et al., 1972; Kass and Tsien, 1975 ; . Both APD50 and APD90 shortened in endocardial muscle, suggesting depression of slow current without significant effects on repolarizing currents. TTX-induced shortening of APD in Purkinje fibers with minimal effects on APA or dV dtmax has been described previously Dudel et al., 1967; Coraboeuf et al., 1979 ; and may be due to suppression of a TTX-sensitive plateau current Gadsby and Cranefield, 1977; Attwell et al., 1979 ; . Increases in phase four depolarization during TTX exposure also have been described previously Vassalle and Scida, 1979 ; , but the mechanism for this phenomenon is not well understood. The changes induced by TTX also support the presence of decreased intrinsic responsiveness in epicardium vs. endocardium, since RMP changes produced by TTX were similar in the two cell types, yet the amplitude of the action potential upstroke during phase zero was attenuated more in epicardial cells, as was dV dtmax- ACT in the two tissues was similar, however, possibly due to depression of Purkinje-mediated conduction in endocardium. Its control lever was used for alignment, adjusting light reflexes away from the disc by slight turning and tilting of the contact lens, and maintaining focus by avoiding the movement of these small animals that a finger-held contact lens can cause.
