C. Baselinecorticoid abnormalities The diagnosis of ectopic ACTH syndrome is based on the demonstration of elevated cortisol and ACTH levels and confirmed by subsequent dynamic tests: 1. Plasma Cortisol is often, but not always, elevated in view of the pattern of adrenal secretory bursts. Intrapartum control of hypertension The fluctuations and spikes of hypertension during labour can be prevented or minimised by antenatal treatment with methyldopa. For patients scheduled for induction of labour and in whom drug treatment has not been commenced, a loading dose followed by three or four doses of methyldopa given during the day before induction is beneficial. If available, epidural analgesia is a useful means to control blood pressure during labour and often makes additional hypotensive agents unnecessary. If the blood pressure is still unsatisfactory after epidural analgesia, hydralazine titration can be used but bolus doses are best avoided as the cumulative vasodilator effects together with sympathetic blockade can lead to marked peripheral pooling of blood and acute fetal distress. For the third stage of labour, ergometrine or its analogues should be avoided and intravenous injections of oxytocin are sufficient to ensure placental separation and prevent postpartum haemorrhage. In the puerperium, antihypertensive treatment should be continued for at least 24-48 hours if it is given before or during labour. This will prevent the unpredictable fluctuations of blood pressure that are often associated with complications like post and metolazone.

Zealand College of General Practitioners' Dunedin Research Unit established its computer research network.2 Before the network conducted any research, the proposal was reviewed and approved by the local ethics committee. Subsequently, each proposed project has been subjected to review by the committee. The data contained on the network are anonymous: no names, addresses, or telephone numbers are ever seen by researchers. Later in the network's development, the unique alpha-numeric code used by the New Zealand public health system was attached to many files. Through a complicated process it was initially possible to identify individual patients using this code. When this was recognised, a further step was added to ensure that researchers received only encrypted identifiers. The de-encryption program is held in a central government agency and the research data are held in a university. But where does this leave the patient's consent? Patients are informed by notices in the offices of practices affiliated with the research network that information from their consultation, investigation, or referral may be used for research once it has been stripped of identifying data. Patients are assumed to have given consent if they continue to be seen in practices affiliated with the network. So effective are the anonymisation processes that individual patients cannot be identified, even to withdraw their data. Data from whole practices can be excluded from research projects but data from individual patients cannot. This is not a perfect system. It requires patients to act in accordance with the social responsibility they incur when they use public health systems: that is, to accept responsibility to contribute to research that advances medical knowledge. It requires researchers to act responsibly in the public interest rather than promoting the commercial interests of health related industries. It requires ethics committees to have a breadth of understanding of issues in medical research that is difficult to achieve when there is short term committee membership and high turnover. On joining the vessel, cadets are introduced to the aims and objectives of the course and what is expected of them, after which they are divided into two groups for familiarisation with the ship's emergency equipment, basic training in shipboard procedures and safety drills such as lifeboat launching, fire fighting and Sopep drills. A comprehensive course manual covers subjects such as health and safety policy and practices, basic safety rules, shipboard practices for all personnel, minimum personal protective equipment, including buoyancy aids, and detailed safety procedures including entry into enclosed spaces, hot work in enclosed spaces, electrical safety, and reporting and logbook recording for engine maintenance, testing of steering gear and other engineering tasks, and bunker operations. At the end of each course, the cadets sit a written examination set by the Chief Officer and oral examinations are conducted by the assigned officer and the Chief Engineer, after which they are awarded certificates by the Master.
