If such improvement has similar positive effects on patient outcomes [9, 10]. Earlier work on the effect of guidelines on patient outcomes concluded that there was little evidence that clinical guidelines are effective in improving patient outcomes, although the poor quality of the guidelines investigated was believed to have had a major influence on this finding [11]. More recent work has indicated that asthma patients receiving guideline-recommended drug therapies have less hospital admissions [12, 13] and better lung function [14] than patients not treated according to the guidelines. What remains unknown is the effect of guidelines on the patient9s asthmarelated quality of life. In this study, health-related quality of life HRQL ; in asthma patients managed in general practice who are treated according to the NIH asthma guideline were compared with the HRQL in those not receiving guideline recommended treatment.

Turnover and an additional study evaluated evidence of the inhibition stopping ; of conversion by protease inhibitors of vitamin D to the more active form [1, 25 OH ; 2 vitamin D3], which is needed for bone formation. Additionally, Dr. David Nolan from the Western Australia HIV cohort showed higher rates of osteopenia bone thinning ; and osteoporosis increased bone softening and loss of bone tissue ; in individuals on protease inhibitors. In the area of avascular necrosis death of bone tissue associated with circulation problems ; , a small group of 14 patients with this disorder was found to have the association of previous Pneumocystis carinii pneumonia infection, prior corticosteroid use not anabolic steroid use ; and low CD4 Tcells less than 50 ; . Lastly, Dr. Pablo Tebas recently published a report in AIDS 2000; 14: F63-F67 ; of a study of 112 patients: 64 received protease inhibitors, 36 HIV-positive patients were not exposed to protease inhibitors and 22 HIV negative persons were used as controls. Various tests were performed to assess bone density, including a specialized x-ray scan DEXA ; to detect bone density reduction. The results of the study showed that 50% of the patients from the group of protease inhibitors use had lower bone mineral density. This was compared with the 6% of the HIV negative controls and 11% of the other HIV-positive patients with no prior protease inhibitor use who showed reduced bone density. While the rates of bone disorders reported here are shocking, one should question how the patients were selected for the study. Most HIV treatment clinics have not wit and rifabutin.

| This paper presents the background to the development of the implementation plan, objectives of the plan, strategies that will be used to implement the activities in the plan. It also presents activities implemented so far, sources of funds to implement these activities, challenges, lessons leamt and the way forward. 15006 ORAL ; THE DISTRICT HIV AIDS GRANTS PROGRAMME IN AIM Eretu Godfrey, David Mabirizi, Denis Ahairwe BACKGROUND: The revised NSF for HP AIDS activities in Uganda repeatedly highlights resource constraints as major impediment in the provision of HIV AIDS services in Uganda. Uganda is facing a crisis in care and the demand for treatment and palliative care services by patients with HIV AIDS. This demand is expected to farther increase as these services become more available. Patients with HIV AIDS related illnesses occupied more than 55% of the Hospital beds in 1997 and by 2000 the occupancy rate had increased to 70%. PNFPs NGOs ; are faced with funding and other capacity and material constraints in providing community and home based care. The limited number of agencies providing psychosocial support are understaffed and under funded to meet the growing demand for these services.' It also emerges that the planning, resource mobilization and resource management capacity in NGOs and FBOs is very weak and yet these are vital actors in the fight against HIV AIDS. Grants have been channeled to local government and district based organizations by different mechanisms depending on the funding agency or donor. The binding agencies at the district level include bilateral and multilateral partners. AIM supports HIV AIDS service delivery in 16 districts of Uganda and includes a grants programme, that works along the UAC District level guidelines that recommend the formation of the District AIDS Committees to coordinate activities at the decentralized level. The DAC appointed Focal Persons are key in guiding and coordinating the grants mechanisms at the district level. DESCRIPTION: The AIM programme is designed to achieve the objectives through technical assistance and grants extended to government services, NGOs, CBOs and FBOs. hi order to achieve this goal AIM works with partners at the district level, in particular the District HIV AIDS Committees DACs ; to assist them in determining unmet service delivery needs gaps ; by working with local planning bodies and identify programmatic approaches by which to address them. The Grants program began in January 2003 and to date funded 251 sub-projects grantees ; . The funding ranges between USD $ 2, 500 to $ 150, 000 witfi an average of USD $ 20, 000. 30% of the grants were extended to Government agencies Government both at local and central levels ; and 70% to Non-Government Agencies 30% Community Based organizations, 10% Faith Based Organizations, 7% Non- Government Oryanizations md3%PrivateService Orsani. EXPERIENCES: The targeted solicitation of proposals process resulted in 587 proposals received by the 16 DACs. 5% of die proposals received and reviewed by the DACs were rejected on the basis of appropriateness, technical soundness and capacity to implement programmes. 38 % of the proposals were rejected by a technical review team consisting of a DAC member. The output of the process was a funding approval rate of 27% 157 ; . Of the 251 grants, 75 programmes have Home based care, 75-OVC, 16-Capacity Development to the DACs, 15-for Opportunistic infections, 20-TB, 31 grants for VCT clinical grants to the DDHS cover a number of sites supported ; . On-going technical assistance financial and programmatic ; to all grantees was carried out by the regional teams and headquarters managers over a two-year period. As a result, the 4 months period absorption capacity of funds on account liquidation rate ; for the 3 months planned activities increased by 25% from 42.1% in December 2003 Nothern-25%, Western-69.5% and Eastern-31.9% ; to 52.6% in December 2004 Nothem-51.3%. Western-59.5% and Eastern-47.1% ; . This funds absorption rate is however still low for effective implementation of activities to people in dire need of services. CONCLUSION: It is important to note that ongoing technical support can improve the implementation of activities and die absorption capacity of grantees. Mechanisms to improve appropriate absorption of funds should be designed to ensure continuity in HIV AIDS service delivery at facility and community level. Donor and Government impediments to utilization of funds markedly affect implementation of activities. They should therefore be regularly reviewed and minimized. This grants programme has a locus on local government involvement DAC ; through the focal persons. By strengthening the existing structures, ownership, supervision and implementation are ensured by the local authorities. The District HIV AIDS focal persons have been key coordinators of HIV AIDS programmes in the Districts and through and with them results can be achieved. 15007 ORAL ; USING A BOTTOM-UP RESPONSE TO TACKLE HIV AIDS Bayiga Milly Katongole BACKGROUND: Concern Worldwide usually referred to as Concern ; is an Irish based NGO, which was founded in 1968 and is devoted to the relief, assistance and advancement of people in need in less developed areas of the world. The agency currently works in 28 developing countries worldwide. In Uganda, Concern operates in six districts of.

Work by blocking the action of acetylcholine and so facilitate the function of dopamine cells in the brain and ritonavir.
Zimolo et al., 1995 ; , leading to a cessation of bone resorption and the induction of apoptosis Hughes et al., 1995 ; . At the molecular level, nitrogen-containing bisphosphonates block the mevalonate biosynthetic pathway Luckman et al., 1998 ; , thereby reducing the synthesis of farnesyl diphosphate and geranylgeranyl diphosphate, essential substrates for the prenylation of small GTP-binding proteins such as Rab, Rac, Ras, and Rho. These proteins are also important components in signaling pathways involved in endothelial migration. Our demonstration that ZOL inhibits angiogenesis provides an additional potential mechanism for the anti-resorptive activity of bisphosphonates. Indeed, osteoclastic bone resorption requires efficient vascularization by the hemangiogenic endothelial cells. Recently, it was shown that two bone matrix proteins involved in bone resorption, osteopontin and bone sialoprotein, stimulate angiogenesis Bellahcene et al., 2000 ; . ` Long-term cancer trials with bisphosphonates such as ZOL indicate that, in addition to reducing bone resorption, some of these drugs may also influence tumor burden and even prolong survival in some patient subgroups Berenson et al., 1998; Diel et al., 1998; Hortobagyi et al., 1998 ; . The mechanism of action responsible for these antitumor properties of bisphosphonates is not yet established and clearly warrants further investigation. Recent in vitro data indicate that the more potent nitrogen-containing bisphosphonates can inhibit the proliferation of human tumor cell lines and induce their apoptosis Senaratne et al., 2000 ; . Angiogenesis, which is essential for tumor growth, could also be a potential mechanism by which bisphosphonates affect tumors but this possibility has received little attention. Our results now demonstrate potent, antiangiogenic properties of the bisphosphonate ZOL. Initial experiments investigated the effect of ZOL on the proliferation, migration, and adhesion of HUVEC in vitro. Cell proliferation induced by serum, bFGF and rhogam.
Do a pilot program first to see how it is received. A pilot is simply a test-run to measure the effectiveness of materials, identify potential problem areas, and gauge participant and community reactions before a project is done on a larger scale. Pilot programs can be done in many different ways. For example, if you are introducing sexuality education into a school or school system, you could start by teaching only one class or grade level in a school or by teaching a course in the community before starting in the schools. During the pilot, educators should be actively observing and recording feedback and rituxan.
